Welcome to Sabuche Health. By accessing or using our telehealth platform, you agree to comply with the terms and conditions outlined below. Please read them carefully.
1. Services Provided
Sabuche Health offers telehealth medical consultations for individuals ages 18 and older. Consultations address focused, non-emergency health concerns suitable for telemedicine and are billed at a flat $65 self-pay rate per session. Sabuche Health does not accept or bill insurance.
2. Provider Licensing & Geography
Sabuche Health providers are licensed to practice medicine in Wisconsin, Iowa, Arizona, Oregon, and Washington, D.C.
You may only book and receive telehealth services if you are physically located in one of these jurisdictions at the time of your consultation.
Consultations outside these locations are strictly prohibited, and Sabuche Health reserves the right to verify your location.
3. Appointment Terms, Fees & Cancellations
All appointments must be scheduled via our secure online portal and are confirmed upon receipt of payment.
Payment is non-refundable except as required by local law or at the sole discretion of Sabuche Health.
Appointments must be canceled or rescheduled at least 24 hours in advance to avoid forfeiture of fees. No-shows or late changes may result in a full charge.
4. Patient Responsibilities
You must provide full, accurate, and current health information relevant to the consultation.
You are solely responsible for the privacy and security of your session environment.
You must not record or broadcast the session without explicit written provider consent.
You agree to use the technology as instructed and notify Sabuche Health promptly of any technical issues.
5. Informed Consent
Telehealth involves the delivery of healthcare using electronic communications, information technology, or other means between a provider and a patient who are not in the same physical location.
Before commencing the session, you will be provided with information regarding the nature, scope, and limitations of telehealth, and you must provide informed consent electronically.
Your provider may determine that telehealth is not appropriate for your situation and may refer you for an in-person evaluation.
6. Scope & Limitations of Care
Telehealth is not suitable for medical emergencies. If you are experiencing a medical emergency, call 911 or visit the nearest emergency department immediately.
Providers may only prescribe medications and make recommendations in accordance with the regulations of the state or district in which they are located during your visit.
The inability to perform certain physical examinations may limit the provider’s ability to diagnose or treat certain conditions.
7. Privacy & Confidentiality
All health information provided and generated during your telehealth encounter is confidential and protected in accordance with HIPAA and applicable federal, state, or district law.
Sabuche Health uses secure, encrypted video and audio technology for all sessions. Despite these safeguards, unauthorized access can still occur, and Sabuche Health cannot guarantee absolute confidentiality.
You have the right to access, review, and request corrections to your medical records.
8. Medical Records & Documentation
Sabuche Health maintains a secure, electronic record of your visits in compliance with all relevant privacy and record retention laws.
Records include health history, consultation notes, diagnoses, treatment plans, and communication between you and your provider.
9. Technology Requirements & Limitations
Telehealth services require a reliable internet connection, a compatible device (with a camera and microphone), and access to our platform.
Technical failures, software malfunctions, or connectivity issues may disrupt service. Sabuche Health assumes no liability for delays, interruptions, or failed sessions due to technical issues.
10. Use Restrictions
Sabuche Health’s telehealth platform is intended for personal, non-commercial healthcare use only.
Any form of abusive, illegal, fraudulent, or inappropriate use of the platform may result in immediate termination of services.
11. Limitation of Liability
Sabuche Health, its providers, affiliates, and contractors will not be liable for any indirect, incidental, or consequential damages arising out of your use of the telehealth platform or services.
Total liability for claims related to telehealth services shall not exceed the amount paid for the session in question.
12. Governing Law
These Terms and Conditions are governed by the laws of the state or district in which you receive services. For Wisconsin, Iowa, Arizona, and Oregon, the respective states’ laws apply; for Washington, D.C., the laws of the District of Columbia apply.
Any disputes will be resolved according to the laws of the location where services were rendered. Sabuche Health encourages resolution through informal negotiation before legal action.
13. Amendments
Sabuche Health may update these Terms and Conditions at any time. Notice of material changes will be provided via the website or email.
Continued use of Sabuche Health services after revisions are effective constitutes acceptance of the revised terms.
